NATIONAL PUERTO RICAN DAY Parade Practices & Performance
in NYC May 1, 2018 to June 10, 2018

There is no financial compensation provided by the “NATIONAL PUERTO RICAN DAY PARADE, Inc” to any involved with our production, therefore we are all volunteering our time. It is for artistic expression, education and fun.

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape in public places, with parents, and/or multiple adults present, without payment or any other consideration, except for appreciation. I understand that my image may be edited, copied, exhibited, published or distributed. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording, because I understand that it will be used to help create a scholarship fund to assist the MusicTech Kids™/MusicTech Teens™ training program. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.

Photographic, audio or video recordings may be used for the following purposes:_• Fundraising, conference presentations, educational presentations or courses, informational, presentations, on-line educational courses, educational videos

By signing this release, I understand this permission signifies that photographic or video recordings of me or my child, may be electronically displayed via the Internet or in the public educational setting. All will be done in public settings with me or other adults present. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed. This release applies to photographic, audio or video recordings collected as part of the sessions listed on this document only. I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any, and all claims against any person or organization utilizing this material for educational purposes.

Liability: I release One World Artists, LLC & David Allan Musial dba MusicTech Kids™/MusicTech Teens™, as well as The Rosados Family & St. Mary’s RC Church in Jersey City (practice location) , from any liability, in the event that my child gets injured on the public property. (If they should trip or fall on their own behalf.)
